Water Filling Machine FAT Report Template
A machine-running video is not a complete acceptance report. Use this editable record to connect one PET still-water monoblock test to its agreed criteria, raw observations and unresolved actions.
A useful filling-machine FAT report identifies the tested configuration, bottle and cap, counting boundary, elapsed time, quality evidence and open actions. Record the result of each agreed check; leave missing evidence unresolved rather than treating it as a pass.
Turn FAT observations into a traceable result
Identify exactly what was tested
Enter the project and report revision, machine serial/configuration revision and bottle/cap sample lots. Link the approved drawings. Record substitutions or unavailable interfaces so this result cannot be mistaken for evidence of another format or a complete line.
Keep the count and clock on one basis
Reference the agreed protocol and criteria. Record the count location, start/end timestamps and counter evidence. Good BPH equals (total minus rejected bottles) × 60 divided by elapsed minutes. Include interruptions in elapsed time; describe stops separately. Deduct each rejected bottle once, not once per defect.
Attach evidence, not only a summary
Link individual fill and closure measurements, bottle observations and instrument records to the agreed limits and sample plan. A good-output calculation does not prove filling accuracy, seal quality or safe operation. Have authorized people record safety checks using the agreed safe procedure.
Keep unresolved actions visible
For each deviation, assign an action ID, owner, due date and supporting evidence. Link a retest to the original run and identify any changed samples or settings. Record the authorized human disposition and signatures separately; this template does not approve shipment or replace site acceptance.

Questions to settle before requesting a quote
Is this a completed FAT report or a certified protocol?
No. It is an editable planning and recording aid for a PET still-water filling monoblock. Agree the actual protocol, duration, limits and sign-off authority before testing. It contains no invented project results or certification.
Should I subtract stoppages before calculating good BPH?
This worksheet uses the full elapsed observation time, including stops. Keep any separately agreed running-time metric clearly labeled and retain both clocks; do not silently replace the elapsed-time result.
Can a passing FAT report prove complete-line output?
No. The report covers only the identified machine, format, conditions and count location. Unavailable upstream/downstream equipment and destination utilities must be disclosed. Site and complete-line acceptance require their own agreed evidence.
